Online Training (Required)
Forms
Completed forms must be returned to the St. Dysmas office, in order to be submitted for processing three weeks prior to the visit.
Department of Corrections (DOC) Access Forms and the online training are required once per calendar year by each visitor planning to attend a prison service. If you have competed the forms in the last year, you may simply compete the Chapel Visit Request Form.
